Crouse Community Center

Crouse Community Center is seeking dedicated Full-Time or Part-Time Licensed Practical Nurse Supervisors to join our team. This role is essential to ensuring high-quality, compassionate care for our residents in partnership with the Unit Manager and under the supervision of the DON and Physicians. You will guide and support CNAs and other unit staff, exercise sound clinical judgment, and maintain strong communication with supervisors and families.


We’re looking for someone with a strong work ethic, excellent communication skills, and a warm, customer-service approach to resident care.


Starting Rate: $28.00

Shift Differentials $4.00 (2nd & 3rd shift)

What You’ll Excel At

  • Delivering compassionate, resident-centered care with professionalism and empathy
  • Collaborating with the interdisciplinary team (Physicians, RNs, CNAs, Ward Clerks)
  • Using strong clinical judgment to monitor resident conditions and respond to changes
  • Communicating promptly with RNs and Physicians regarding resident status
  • Demonstrating leadership when delegating tasks and ensuring care plans are followed
  • Maintaining accuracy in medication ordering, documentation, and charting
  • Assisting with wound care, vital signs, and treatment procedures
  • Supporting residents with personal hygiene and maintaining a clean, safe environment
  • Coordinating appointments and communicating with families
  • Entering orders, notifying pharmacy, and maintaining thorough records
  • Participating in daily meetings, care conferences, and discharge planning
  • Following all policies related to controlled substances and documentation
  • Reporting safety concerns, facility issues, or emergencies promptly
  • Reporting suspected abuse, neglect, or mistreatment in accordance with regulations

Requirements:

Required Qualifications

Education

  • High School diploma or equivalent
  • Completion of an accredited Licensed Practical Nurse program

Licensure

  • Current New York State LPN license

Certifications

  • BLS/CPR (Preferred)
