The job will be 20 hours per week, split between Center Line and Mt. Clemens locations.
GENERAL DESCRIPTION:
The Registered Dietitian provides comprehensive nutrition services within a community health center setting. This position supports children, adults, and families through evidence-based Medical Nutrition Therapy (MNT), with a strong emphasis on chronic disease management including diabetes, hypertension, obesity, and cardiovascular disease. The RDN works collaboratively with the clinical care team to deliver individualized nutrition counseling, promote behavior change, and support overall health outcomes. The role also includes appropriate documentation and billing for nutritional services.
SPECIFIC DUTIES AND JOB FUNCTIONS:
Clinical Nutrition & Counseling
1. Provide individualized Medical Nutrition Therapy (MNT) for patients with chronic conditions.
2. Conduct comprehensive nutrition assessments including diet, labs, and medical history.
3. Offer nutrition counseling for children and adults, addressing a range of health needs.
4. Develop individualized, culturally appropriate care plans.
5. Monitor patient progress and document outcomes.
6. Compile statistical data needed to complete reports required by the program or organization.
7. Develop nutrition education materials, newsletters, and other outreach materials.
Team-Based Care & Collaboration
8. Participate in coordinated care with clinical staff.
9. Contribute to case conferences and chronic disease programs.
10. Serve as a nutrition resource for staff.
Documentation, Coding & Billing
11. Document all encounters in the EHR.
12. Use appropriate MNT and nutrition billing codes.
13. Ensure compliance with payer documentation requirements.
Education & Community Engagement
14. Develop and facilitate nutrition-related classes and workshops.
15. Support health center outreach efforts.
16. Participate in quality improvement initiatives.
17. Make referrals to appropriate community organizations as needed.
18. Collaborate with community partners, public health agencies, and businesses to increase area resources related to nutrition.
