Summary of Job Duties:BENEFITS ELIGIBLE: The RN will float to outpatient primary care clinics within the service line as needed, 8-hour shifts vary from 0730-1800. Part-time position. 20+ hours per week with benefits. Outpatient Monday - Friday.
The Registered Nurse will promote wellness; improve outcomes, and self-care within a defined population of patients. The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state. Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. Commitment to patient outcomes results in high levels of performance despite workload changes.
Qualifications:

Minimum Qualifications:

  • Bachelor's degree in Nursing, Associate's degree in Nursing, Diploma RN, or Master's degree in Nursing.

  • Must be a graduate of an accredited RN nursing program

  • Valid Registered Nursing License

  • Minimum of 1 year of Registered Nursing experience;

  • Basic computer skills.

  • Excellent communication skills.

  • Strong work ethics and accountability.

Responsibilities:

  • Collects and documents assessment data from patients and/or others per UAMS standard.

  • Develops patient's Plan of Care (POC) per department/unit standards utilizing assessment data.

  • Evaluates the progress of patients toward attainment of desired outcomes.

  • Administers medication following department/unit standards and the Nurse Practice Act.

  • Verifies and evaluates care delivered to ensure it is consistent with patient and family-centered care principles, practice standards, and regulatory requirements.

  • Supports the integration of current evidence into nursing practices.

  • Promotes an environment conducive to positive educational experience for all students and staff.

  • Provides effective patient/family education/discharge planning.

  • Effectively collaborates and communicates with nursing staff at all levels, interdisciplinary teams, executive leaders, and other stakeholders.

  • Communicates care appropriately through documentation in the medical record.

  • Reports variances in care/ systems/ processes through the appropriate means.

  • Participates in the recruitment/retention of staff.

  • Facilitates the development and implementation of nursing practice standards that are evidence-based and in compliance with ANA standards.
