Job description

ESSENTIAL FUNCTIONS OF POSITION: • Ensure all residents are assessed weekly to evaluate the condition of their skin; identify risks and develop appropriate interventions and required documentation. • Measure all skin areas weekly and document. • Initiate and update all care plan information regarding skin. • Follow up with physician to report any resident skin problems that arise and transcribe and follow up with any new orders. • Perform skin and related treatments on all residents as assigned, according to Physician’s orders and in accordance with professional standards. • Communicate with the physician, residents, family members, charge nurse, other interdisciplinary team members, and facility management regarding resident status as appropriate. • Chair the Skin Management meeting on a weekly basis and provide needed information to the inter-disciplinary team to develop appropriate interventions to prevent avoidable skin breakdown. • Monitor and adjust equipment use per facility protocol and resident need. Keep an ongoing record of all facility equipment and use. Be available for review of new and upcoming devices that can promote healing and reduce risk of infection. • Follow skin protocol of facility and F314. • Be able to provide weekly skin log, utilize computer programs available, and take responsibility for the prevention, healing, and educating of residents, families, and staff in areas of need according to F314 and facility protocol. • Participate in Quality Improvement activities as assigned. • Perform additional duties as assigned.

EDUCATION/REQUIREMENTS: RN license preferred. (LVN/LPN by exception only) plus a minimum of 3 years related experience; or equivalent combination of education and experience. Must be in good standing and currently licensed by the State. Must be able to relate positively and favorably to residents and families and to work cooperatively with other associates. Must be organized and efficient in order to maximize tasks and educate staff where appropriate. Must be able to perform all duties required to direct and manage the skin program for the facility.
