Summary:

Meet psychosocial needs of patients and families through clinical assessment, crisis intervention, supportive counseling, case management, and case consultation with integrated health care team and collaboration with community professionals.

Job Responsibilities:

Clinical

  • Assesses patient and family needs and coordinate, advocate and problem solve to enhance their functioning.
  • Completes psychosocial assessment, including the gathering of social, psychosocial, cultural and financial information to determine level of support the family requires.
  • Develops plan based on assessment and assists family in implementing plan (referrals, follow up calls, DCF reporting).
  • Practices as part of an interdisciplinary, inter-professional team, accepting "warm hand offs" from primary care providers and other members of the health care team and referring to appropriate services within the agency and the greater community.
  • Attends clinical Team Meetings as assigned and Component Meetings.
  • Attends clinical supervision or seeks out consultation or supervision as needed.
  • Meets all CHC standards as outlined annually during employee's performance appraisal review with a focus on quality clinical care.

Administrative

  • Completes all documentation required by state regulations, Joint Commission standards, and CHC Behavioral Health department by laws and CHC policies and procedures that are appropriate for this position in a timely manner.
  • Completes and locks all WHO notes no later than the end of the next business day.
  • Completes reports as requiredfor outside agencies such as DCF, Family Relations, probation, and others with appropriate releases signed by clients.
  • Participates as a member of agency wide committees as assigned.

Required Experience & Qualifications:

  • MSW required; LCSW preferred.
  • Minimum of two years of related experience with children and families.
  • Written and oral bilingual fluency in Spanish required

Physical Demands & Environment & Work Schedule Expectations:

  • Combination of sustained periods of sitting at keyboard, walking and stair climbing.
  • Minimal lifting and carrying activities.
  • Must be available to work one evening a week and one Saturday per rotation.

Communications Skills:

  • Excellent oral and written communication skills are required.
  • This position must interface with patients, family members, community agencies and other providers within the organization.

  • Behavioral Health information is kept in strict confidence.Confidentiality of patient and business information is a requirement. Full access to patient medical records and encounter data. Confidentiality must be maintained according to CHC policies.
