Job Title: SupportiveCare Nurse Status(Full-time, Part-time, PRN): Full-time
Department: ResidentCare Reports to(position): Health Care Administrator
Pay Type (Hourly/Non-Exempt orSalary/Exempt): Non-Exempt
Location: Homecare
Job Overview:
In this role, you will oversee PCA's,Med Tech's, and CNA's ensure high quality resident care, and maintaincompliance with all Homecare regulations. The Supportive Care Nurse willcoordinate and supervise daily clinical operations, provide direct residentcare as needed, and support a positive work environment for staff andresidents.
Responsibilities:
ResidentAssessments & Plans of Care
  • Conductcomprehensive initial nursing assessments for all home care clients
  • Developindividualized Plans of Care in compliance with NC DHSR standards
  • Reviewand update Plans of Care at least every 90 days or when a change incondition occurs

Supervisionof Aides
  • Provide initialand ongoing supervision of Personal Care Aides, Med Techs, and PCAs
  • Conductsupervisory visits at least every 90 days for personal care services and every60 days for nursing tasks delegated to unlicensed staff
  • Documentsupervisory visits and maintain compliance records

Delegation & Oversight
  • Delegatenursing tasks appropriately to unlicensed personnel in accordance with NCBONdelegation rules
  • Ensureaides and med techs perform care in line with Bermuda Village policies,training, and state requirements
  • Provideguidance, mentoring, and support to the home care team

Compliance & Quality Assurance
  • Ensureservices are delivered in accordance with 10A NCAC 13J, HIPAA, and BermudaVillage policies
  • Participatein quality assurance/performance improvement (QAPI) programs
  • Assistleadership in preparing for audits, surveys, and regulatory reviews

Communication & Coordination
  • Serve asliaison between physicians, residents, families, and the Bermuda Village careteam
  • Provideeducation to residents and families regarding the scope of home care services
  • Communicateeffectively with staff and leadership regarding care delivery and residentoutcomes
  • Supervise teams of PCA's and Med Techs to ensurepatient care needs are being met
  • Keep proper records of care as required by thefacility. Complete all forms and documentation as required
  • Assessing and monitoring wounds; cleaning andbandaging wounds
  • Working collaboratively with other members of thepatient care team to determine if additional treatments or changes in care arerequired
  • Immediately report any observations ofconcern/changes in residents' condition to the Health Care Administrator, orOn-call providers, per physician group (some residents may have differentproviders)
  • Vital sign monitoring will be performed as neededor directed by regulations, policy or primary care provider's orders
  • Be able to apply the community's accident,disaster, fire safety, elopement and emergency procedures for protection ofresidents
  • Be capable of performing CPR upon successfulcompletion of CPR Certification
  • Be knowledgeable of the residents' background,preferences, habits and interests and preserve confidential information abouteach resident
  • Understand and follow safety and infection controlpolicies and procedures and readily report
  • Respond to emergency call signals promptly
  • Report incidents to the Health Care Administratorpromptly and complete appropriate paperwork as required in a professionalmanner
  • Readily and directly report any verbal or physicalabuse of a resident, inappropriate use of resident or facility property or lackof or inappropriate care of a resident to the Health Care Administrator
  • Help to properly maintain all equipment and ensurework areas are clean and safe
  • Assist in cleaning residents' rooms, dining areas,and public areas as indicated by regulations
  • Ability to communicate effectively with residents,families, staff, vendors and the general public
  • Must be willing to be on-call in rotation with ourAdministrative Nursing Team
  • Must have compassion for and desire to work withthe elderly and understand that for each resident the facility is consideredthe Resident's home
  • Must demonstrate the ability to work responsiblyas a team member as well as an individual
  • Must be honest, ethical, fair, dependable, respectconfidentiality and the rights and privacy of others
  • Responsible for ensuring that all employees areproviding excellent customer service to internal and external customers
  • Perform job duties for residents and with teammembers in a courteous and professional manner
  • Take the initiative to ensure resident safety andsatisfaction is a priority
  • Meet and greet visitors in a friendly, helpfulmanner (visitors include anyone who visits the community; specifically, currentresidents' families and friends, prospective residents and their families,referral sources, vendors and regulators)
  • Answer phones appropriately, according to companyand community standards
  • Communicate professionally and cooperate withsupervisors and all community personnel
  • Must be able to relate to and work with the ill,disabled, elderly, emotionally upset and, at times, hostile people within thecommunity
  • Ensure compliance and understanding of allregulations regarding residents' rights
  • Perform other duties assigned

Required Qualifications:
  • Current RegisteredNurse (RN) license in good standing with the North Carolina Board ofNursing
  • Minimum 1year of nursing experience (home care, long-term care, or community healthpreferred)
  • Knowledgeof NC DHSR home care regulations (10A NCAC 13J) strongly preferred
  • Demonstratedability to supervise, delegate, and support unlicensed personnel
  • Strongorganizational, leadership, and communication skills
  • Must meet all health and safety requirements
  • Must pass a criminal background check
  • Must be able to perform duties andresponsibilities, with or without reasonable accommodation

Preferred Qualifications:
  • Pervious supervisor experience is preferred

Physical Requirements & WorkConditions:
  • Must be willing to be on-call in rotation with ourAdministrative Leadership Team
  • Schedule hours; possible overtime. May work inother positions temporarily when necessary
  • Abilityto conduct supervisory visits throughout the community campus
  • Occasionalneed to lift or move objects up to 25 lbs
  • Flexibleschedule to meet regulatory and resident needs
  • Is subject to callback during emergency conditions(e.g., severe weather, evacuation, post disaster, etc.)
  • Is subject to injury from falls, burns fromequipment, odors, etc., through the workday, as well as to reactions from dust,disinfectants, and other air contaminants
  • May be subject to the handling of and exposure tohazardous chemicals
  • Including but not limited to sitting/standing inplace for short/long periods at a time, repetitive use of hands, fingers,wrists, arms, frequent walking, bending, reaching, stooping, squatting,pushing/pulling/lifting 25+lbs. High people interaction, bright and lowlighting, loud noises, fluctuating temperatures, exposure to odors communicableillness/disease and other related infectious contagions found in ahealthcare/residential setting
