About the Role

As a Healthcare Advocate at Baba, you will play a vital part in enhancing the healthcare experience for our clients. Using your care advocacy skills and experience, you’ll offer guidance and support to patients and their families—helping them access care, clarify provider instructions, connect with community resources, and reduce barriers to a healthier, more independent life.

This is a remote, 1099 (contractor) role. You may choose part-time or full-time engagement.

NOTE: Candidates must be available for a minimum of 20 hours/week (Monday through Sunday), including at least 10 hours during weekday standard business hours (9 AM–5 PM local time, Monday through Friday).

What You’ll Do
  • Develop trusting relationships with patients, grounded in listening, respect, and clarity.

  • Assess the whole person — medical, social, and environmental factors — to pinpoint barriers and needs.

  • Create individualized care plans that integrate medical, social, and behavioral supports.

  • Support patients across a range of needs, including:

  • Connecting to local services (transportation, meal delivery, home care, social supports)

  • Analyzing medical bills and helping with claim appeals or explanations

  • Identifying medication cost savings, co-pay assistance, or formulary options

  • Linking to social support networks (caregivers, community programs, peer groups)

  • Organizing and maintaining medical records, care histories, and provider communication

  • Coaching and supporting patients on best practices for managing conditions and navigating care

  • Document interactions, observations, and patient progress in Baba’s systems with clarity and timeliness.

  • Collaborate with clinical, product, and operations teams to optimize workflows, escalate complex issues, and refine the patient experience.

  • Provide feedback on platform features, tools, and processes to improve Baba’s care copilot capabilities.

What You Bring
  • 3+ years’ experience in care management, patient advocacy, case management, or clinical social work (especially working with older adult or Medicare populations).

  • Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals.

  • Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse and underserved populations.

  • Empathy, patience, and strong communication skills— you can explain complex things simply.

  • Comfort working with digital tools, EMRs / care-management software, and adapting to new tech.

  • Commitment to continuous improvement— you’ll raise issues, suggest solutions, and help refine Baba’s model.

  • Organized, proactive, and able to juggle multiple priorities without losing quality.
