Job description

REPORTS TO: Admissions Manager FLSA STATUS: Non-Exempt
SCHEDULE: Part Time, hours vary, could involve a weekend rotation (2 positions available)
POSITION SUMMARY: Responsible for completing new intake processing as well as following up with new referrals in a timely manner. Also, responsible for some transport responsibilities as assigned by the Admissions Manager.
GENERAL RESPONSIBILITIES:
Coordinate Referrals/Intakes, and Transport of Clients Duties:
  1. Works with Admissions Manager/Intake Team to contact all referrals in a timely manner. Provides general information on services offered by Revive programs. Gathers general information on potential client's mental health needs, current level of crisis, payment options, legal status, and other relevant information.
  2. Identifies factors which suggest the referral may be better served by another agency dependent on need and level of care. If appropriate, provide contact information for other agencies.
  3. Schedule applicant for admission date.
  4. Oversee and manage orientation process for each intake.
  5. Process Paperwork, Maintain Records in Timely Manner.
  6. Provides needed documentation to enroll clients into residential treatment.
  7. Coordinates and ensures timely processing of program documents including referrals, intakes, discharges
  8. Determines client's strengths and needs by establishing a safe trusting relationship with clients, families, and caregivers.
  9. Data entry and completion of signing documents into electronic health record with intakes.
  10. Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner.
  11. Complete projects and tasks as assigned by supervisor.
  12. Actively participates in daily/ weekly meetings and annual trainings.
  13. Pick up clients at scheduled locations and times and transport to appropriate destinations in a punctual and timely manner.
  14. Drive safely in accordance with traffic laws and posted speed limits while accounting for weather conditions.
  15. Travel by routes specified by patient transportation manager or an approved GPS system
  16. All other duties as assigned.

BACKGROUND/EDUCATION:
  1. High School Diploma or equivalent (GED).
  2. Experience in the behavioral health field a plus.

QUALIFICATIONS REQUIREMENTS:
Must have knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPAA]); computer applications/software to include Microsoft Office programs/MS Outlook (e-mail), and internet familiarity. Electronic medical records experience is desired.
Having a comfort level working with incarcerated individuals in a jail environment is a must.
Experience working with individuals and families who have various mental, behavioral, and substance abuse conditions is a plus.
SKILLS REQUIRED:
  1. Must have organizational and communication skills, both written and verbal.
  2. Communicate effectively in oral and written format with i.e., attorneys, physicians, and other referring agencies.
  3. Maintain effective interpersonal relationships.
  4. Must have valid driver's license
  5. Must meet insurance provider's guidelines for insurability
  6. Must have ability to work independently and work in a stressful and busy environment.
  7. Office administration phones, copier, scanner, fax machines etc.
  8. Running background checks with confidentiality
  9. Moderate computer skills including but not limited to Microsoft Excel and Word and various web sites.

WORKING CONDITIONS:
Physical Demands:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Must be able to sit, stand, bend, reach overhead and lift to 10 pounds; travel outside of the office and/or attend meetings both during and after normal work hours, both at and away from the work site. Exerting up to 10 pounds of force occasionally or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. May involve walking or standing for brief periods of time. Requires about 6 hours of sitting and no more than 2 hours of standing or walking in an 8-hour workday. No significant stooping is required.
Environmental Conditions:
The employee will work indoors in a residential setting with central heating and air conditioning. The house itself is open and ventilated well with central heating and air conditioning. The person may work outside in heat/cold, wet/humid, and dry/arid conditions during times of community activities for clients.
Mental Demands:
There are several daily deadlines associated with this position, which may cause significant levels of stress for some individuals. The position must deal with the clients, staff, and insurance representatives daily on a variety of issues.
