Responsibilities
  • Complete OASIS Start of Care, Resumption of Care, Recerts, and Discharges
  • Perform head-to-toe assessments and create individualized plans of care
  • Teach patients and families on meds, safety, and disease management
  • Coordinate with physicians and office clinicians for timely updates
  • Document accurately in WellSky Kinnser and meet compliance standards
Qualifications

Required

  • Active Texas RN license in good standing
  • Minimum 1 year recent home health RN experience with OASIS
  • Proficiency in WellSky Kinnser
  • Reliable transportation and valid drivers license
  • Strong communication, organization, and time management
