Blue Ridge Health is currently seeking a part time Community Health Worker - Food Box Delivery Driver to be part of our Community Outreach Team.

What You'll Do:

The Community Health Worker (CHW) is a front-line public health worker that works as a liaison between BRH and the communities they serve. This team member must be a dependable and compassionate Meal Delivery Driver to distribute nutritious food boxes to designated patients within the communities we serve. This role is a vital in ensuring timely and safe delivery of meals, contributing to the well-being of our community members. Responsibilities include:

  • Safely pick up and deliver prepared food boxes to patient’s residences following a predetermined route and schedule.
  • Conduct brief wellness checks during deliveries, noting any concerns and reporting them to the appropriate personnel.
  • Maintain accurate delivery records, including delivery confirmations.
  • Adhere to all traffic laws and company policies to guarantee safe transportation.
  • Connects individuals to health care services.
  • Provides basic health education/coaching to the community in order to prevent negative health outcomes.
  • Provides resource navigation assistance in order to meet individual’s needs.
  • Facilitates and promotes access to services for vulnerable populations, including migrant farmworkers, the homeless, and other underprivileged communities

What We're Looking For:

  • Community Health Worker certification required. If not certified, must obtain certification within 1 year of employment.
  • Must possess a strong passion for community engagement work.
  • Lived experience or prior work experience with homeless populations is preferred.
  • Ability to communicate clearly and professionally in order to facilitate collaboration and cooperation both internally and externally.
  • Must be reliable and flexible in order to meet both the internal and external changing needs of the work.
  • Strong organizational skills and the ability to work autonomously.
  • Effectively manage assignments and projects.
  • Ability to prioritize and respond to multiple requests within requested time frames.
  • A valid driver’s license is required.
  • Strong interest in working with an interdisciplinary, non-profit healthcare team as well as culturally and socioeconomically diverse community and patient population.
  • Proficiency with Microsoft Office programs and Electronic Medical Records.
