What you will do:
  • Complete initial and on-going interdisciplinary assessment of the patient with an emphasis on level of functioning, including OASIS assessments at appropriate time points.
  • Execute assessment using procedures such as manual and electrical muscle examinations, functional evaluations, gait analysis, sensory testing, measurements of range of motion, and assessment of development and utilization of human movement.
  • Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s), and in consultation with the physician and interdisciplinary care team members.
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Provide therapeutic treatments and evaluation of equipment needs, as applicable.
  • Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care, including patient reaction to treatment and any changes in patient condition.
  • Instruct patients in use of equipment, as appropriate, such as wheelchairs, braces, canes, crutches, prosthetic & orthotic devices.
  • Evaluate the home environment and make the appropriate recommendations.
  • Advise, consult, and when appropriate, instruct family and other agency personnel in patient's therapy program.
  • Provide education and counseling to the patient, family, and caregiver(s) in accordance with individualized needs.

Who you are:
  • Education and qualifications commensurate with § 484.115(h) (Condition of participation: Personnel qualifications / Physical Therapist)
  • Licensed as a physical therapist pursuant to ARS Title 32, Chapter 19 by the Arizona State Board of Physical Therapy.
  • Core competency per skills checklist.
  • At least one year of experience in physical therapy.
  • Fingerprint card, DMV and criminal background check, as appropriate.
  • Current CPR, TB and health clearance.
  • Complete 6 hours of in-service training per year .
