Job description

Centurion is proud to be the provider of comprehensive healthcare services Indiana Department of Correction. Centurion Health contracts with state and local governments nationwide to provide comprehensive healthcare services to correctional facilities, state hospitals, and other community settings.
We are seeking a full-time Transitional Healthcare Liaison for needs at the Parole District located in Bloomington, Indiana.
Available Shift: full-time days, Monday-Friday
Pay Rate $25 per hour + benefits!
The Transitional Healthcare Liaison will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Liaison will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources. Short term travel will be required to visit other juvenile facilities within the state of Indiana.

โ€ข Bachelor's degree in psychology, social work, counseling or related field from an accredited institution required
โ€ข Two (2) years of experience working in a correctional setting
โ€ข Experience with/knowledge of community and state programs and services
โ€ข Two (2) years of experience in re-entry services or release planning preferred
โ€ข Must show/obtain current Purified Protein Derivative (PPD) documentation for tuberculosis screening
โ€ข Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR) and Automated External Defibrillator (AED) use
โ€ข Must be able to pass background investigation and obtain agency security clearance where applicable
