Job Summary:
As a Licensed Practical Nurse (LPN) at Paramount Community Living Center & Rehab, you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. You will work in a supportive and dynamic environment where your skills and dedication contribute significantly to the well-being of our residents.
Responsibilities:
โ€ข Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
โ€ข Monitoring and Assessment: Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
โ€ข Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
โ€ข Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
