Theย Mobile Integrated Health (MIH) Paramedicย delivers high-quality, patient-centered medical care in the home setting as part of our Mobile Integrated Healthcare (MIH) program.ย  This role extends clinical services beyond traditional emergency response by providing scheduled, proactive, and post-discharge care to patients in collaboration with healthcare providers.ย

The MIH Paramedic works closely with healthcare organizations, physician groups and accountable care organizations (ACOs) to reduce unnecessary emergency department utilization, prevent hospital readmissions, and improve overall patient outcomes.
ย 
This position requires strong clinical judgment, excellent communication skills, and a passion for delivering compassionate care directly to patients in their homes.
What You'll Do:
  • Perform in-home patient care including but not limited to: Checking vital signs, reviewing medications, reviewing allergies, physical assessments, pain assessments, wound care, intravenous access, medication administration & blood sugar readings.
  • Connect patients with their Primary Care Physician via telehealth video interaction.
  • Collaborate with Physicians, nurse practitioners, patients, caregivers, and interdisciplinary team members.
  • Educate patients and caregivers on preventive healthcare measures, medication management, lifestyle modifications, and community resources available to support their health and well-being.
  • Document patient encounters, assessments, interventions, and referrals accurately and promptly, following organizational policies and regulatory requirements.
  • Maintain licensure, certifications, and continuing education requirements necessary for paramedic practice, staying current with advances in prehospital care and community health practices.
Qualifications:
  • Current Illinois Department of Public Health (IDPH) Paramedic License.
  • Minimum two (2) years of experience working as a paramedic in an EMS or healthcare setting.
  • Demonstrated knowledge of clinical protocols, emergency medical procedures, and patient care standards.
  • Valid driver's license and clean driving record.
  • Position requires working vehicle in good condition.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Commitment to patient-centered care, cultural competence, and ethical conduct.
  • Must pass a background check.
  • Must possess a valid CPR card.
  • Must complete CDH EMS System entry process.
  • Stay current with EMS System requirements.
  • Proficient in Epic & Image Trend (preferred).
  • Must possess uncompromised character, high ethical standards, scholarly concern for improvement, and motivation for continued learning.ย ย 
Physical Requirements:
  • Ability to lift and carry approximately 15-20 pounds.
  • Stamina to work extended shifts, respond to pre-scheduled calls, and navigate various environmental conditions.
  • Visual and auditory acuity to assess patients, interpret diagnostic tests, and communicate effectively.
