Job description

A pain management practice expanding across East Bay skilled nursing facilities is hiring NPs and PAs for geriatric pain and palliative care. You'll follow the same residents weekly, manage pain medications, and build real treatment relationships — meaningful longitudinal work with a population that rarely gets this level of dedicated pain care. New graduates welcome with structured onboarding. Malpractice with tail coverage included. Practice actively adding facilities with room to grow.

Key Responsibilities

  • Conduct thorough pain assessments including history, physical exam, and identifying source and severity
  • Build individualized treatment plans combining medication, non-drug, and interventional approaches
  • Manage prescriptions responsibly per state and federal regulations
  • Coordinate referrals for physical therapy, acupuncture, spinal blocks, and interventional care
  • Guide patients and families through pain management strategies and lifestyle modifications
  • Partner with physicians and facility providers to keep care aligned
  • Track treatment effectiveness through follow-ups and adjust plans as needed
  • Maintain accurate documentation in TEBRA and PointClickCare

Qualifications

  • Active, unrestricted California NP or PA license
  • AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification
  • Fully onsite; comfortable traveling between East Bay facilities
  • New graduates welcome; 5+ years relevant RN experience also counts
  • Background in geriatrics, skilled nursing, palliative care, or pain management preferred
  • TEBRA or PointClickCare experience a plus
  • Active Medicare credentialing strongly preferred

Schedule & Compensation

  • $60–$100/hour + productivity bonus | ~$115,200–$192,000/year
  • Full-time | Schedule varies by facility and caseload
  • Onsite at Richmond and East Bay skilled nursing facilities
  • Malpractice with tail coverage included
  • Structured onboarding program
  • Note: Licensure costs are the candidate's responsibility; no sign-on or relocation
  • Immediate hire
