Position Summary As an independent contractor RN, you will conduct in-home nursing assessments for our Medicaid and private-pay clients. Your clinical documentation directly supports our plan of care process and ensures regulatory compliance under 12VAC5-381. This is a flexible, per-assessment engagement — ideal for an RN seeking meaningful supplemental work on your own schedule.

Key Responsibilities • Conduct initial and follow-up in-home nursing assessments for home care clients • Develop and document individualized written plans of care consistent with client needs • Assess client ADL and IADL status, cognitive functioning, safety, and environmental factors • Identify skilled nursing needs, fall risks, medication concerns, and reportable conditions • Communicate findings and care recommendations to the agency administrator and care team • Complete all required documentation in compliance with 12VAC5-381-360 (Virginia home care regulations) • Conduct supervisory visits as needed, not to exceed 90-day intervals per regulatory requirement • Remain available by phone during agency operating hours for clinical consultation (on-call basis) • Participate in annual caregiver in-service training as requested (12 hours per year per regulation)

Qualifications • Active, unencumbered Virginia Registered Nurse (RN) license — required • Minimum 2 years of clinical experience; home health or home care experience strongly preferred • Familiarity with Virginia home care regulations (12VAC5-381) preferred • Strong assessment, documentation, and communication skills • Reliable transportation for in-home visits across Hampton Roads • Ability to respond to assessment requests within 48–72 hours • Comfort working independently and as part of a small, growing agency team • Current CPR/BLS certification preferred • transportation with a valid driver’s license and proof of insurance • Other Requirements: o Ability to travel locally within assigned service area o Strong documentation and communication skills o Compassionate patient-centered approach to car
