Home Health  /  Registered Nurse (RN)  Clinical Care Manager

Responsibilities:

  • Demonstrate leadership skills in decision making and problem solving.
  • Provide direct patient care for assigned patients on the nursing unit and providing leadership within the nursing team.
  • Accountable for the care to the patient and for the decision regarding which nursing tasks can be safely delegated to licensed and unlicensed persons involved in patient care.
  • Must be competent in a wide range of nursing skills.  Responsible for adhering to all standards of nursing care and promoting a team effort among co-workers.
  • Utilize professional judgment and critical thinking to prepare, prioritize and execute the patient's plan of care consistent with patient assessment data and physician's treatment plan and professional standards of evidence based practice/care.
  • Incorporates the patient's physiological, psychological, cultural, developmental and spiratual needs into the plan of care.
  • Collaborate with all members of the health care team with a focus on promoting evidence based care within a healing culture for the patient population served and their families.

Job Requirements:

  • Education: Graduate of an accredited school of nursing.  ADN required.  BSN or MSN preferred.
  • Licensure:  Current Arkansas or Compact State RN License in good standing.  Valid and clean Driver’s License, reliable transportation and agency required liability insurance to complete home visits.  Active personal automobile liability insurance coverage consisting of $100,000 per person and $300,000 per accident for bodily injury required.
  • Experience: Minimum of one year acute care nursing experience required.  Previous nursing experience in a medical-surgical environment preferred.  
  • Certifications:  BLS required.  
  • Other:  Ability to read, write and communicate effectively.  Basic computer knowledge required.  Driving required.
