Key Responsibilities

  • Continuously monitor and assess patient progress, employing evidence-based interventions and therapeutic strategies to support positive mental health outcomes.
  • Document and maintain precise treatment records and reports using modern technology in compliance with organizational regulations, ensuring timely data entry and data integrity.
  • Collaborate with patients and relevant stakeholders to develop comprehensive, individualized treatment plans that prioritize desired outcomes and actively involve patients in their care journey.
  • Facilitate appropriate referrals and coordinate discussions between patients and treatment providers to ensure a seamless continuum of care.
  • Provide mentorship and education to students, interns, and volunteers, enhancing their professional skills and knowledge in mental health care.

Patient Population / Client Focus

Qualifications

Compensation and Benefits
