Key Responsibilities

As a Licensed Mental Health Counselor, your primary responsibilities will include:

  • Conducting thorough reviews of patient health histories to prepare for initial evaluations, ensuring a comprehensive understanding of their mental and emotional status.
  • Developing, implementing, and continuously reevaluating personalized mental health care plans aligned with our established standards of practice.
  • Engaging in proactive interviews with patients and their families to assess educational needs, set treatment objectives, and design tailored plans of care.
  • Regularly assessing patient and family responses to educational initiatives and modifying care plans based on feedback and outcomes.
  • Collaborating with the patient’s physician and members of the interdisciplinary team to coordinate comprehensive care and effectively communicate patient status updates.
  • Making timely referrals to other healthcare professionals as necessary to address the broader healthcare needs of the patients.
  • Maintaining strict patient confidentiality in compliance with HIPAA regulations while diligently documenting care on required cardiac rehabilitation forms.
  • Leading workshops on mental health awareness, such as “Healthy Minds” sessions, aiming to educate patients and families on maintaining psychological well-being.
Patient Population / Client Focus

The ideal candidate will be dedicated to serving a unique patient population comprised primarily of individuals managing cardiac health issues. A profound understanding of the intersection between mental health and physical health challenges is crucial. You will work closely with patients who are often experiencing anxiety, depression, and other mental health conditions as they navigate their cardiac care journeys.

Qualifications

To be considered for this role, applicants must possess the following:

  • Hold a valid credential as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Psychologist.
  • A Bachelor's degree in a relevant field is required; a Master's degree is preferable.
  • Demonstrated experience in mental health counseling, particularly in outpatient settings or cardiac care is highly valued.
Preferred Experience
