Job description

Make an Impact in Community Health

Community Health Partnership of Illinois (CHP) is a nonprofit healthcare organization providing primary medical, dental, and behavioral health services to migrant farmworkers and other medically underserved populations throughout Illinois.

We are seeking a compassionate and dependable Registered Nurse (RN) to join our Mendota healthcare team. This position is ideal for an RN who enjoys patient-centered care, working collaboratively with providers, and making a meaningful difference in the health of the community.

What You'll Do
  • Assess patients and monitor changes in health status
  • Obtain and document vital signs, medical histories, and other clinical information
  • Provide nursing care according to established clinical protocols and provider direction
  • Administer medications, immunizations, and treatments as ordered
  • Assist providers with examinations, procedures, and follow-up care
  • Provide patient education regarding medications, treatment plans, preventive care, and chronic disease management
  • Coordinate patient care and communicate with providers and other members of the healthcare team
  • Respond appropriately to patient questions and clinical concerns
  • Maintain accurate, timely, and confidential documentation in the electronic health record
  • Support infection-control, quality, and patient-safety practices
  • Help connect patients with appropriate follow-up care and community resources
  • Maintain compliance with applicable clinical, organizational, and regulatory requirements
What We're Looking For
  • Current, active Registered Nurse (RN) license in Illinois
  • Graduate of an accredited nursing program
  • Previous outpatient, primary care, community health, or other clinical experience preferred
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and patient-service skills
  • Ability to work both independently and collaboratively as part of an interdisciplinary healthcare team
  • Strong organizational skills and attention to detail
  • Commitment to providing respectful, culturally responsive care to patients from diverse backgrounds

Bilingual English/Spanish skills are a plus.

Newer RNs with strong clinical training and an interest in community health are encouraged to apply.
