Clinical Case Manager - Behavioral Health (Licensed)

Are you a licensed Behavioral Health clinician looking for a rewarding remote opportunity where you can make a meaningful impact on members' lives? Join our team and use your clinical expertise to support individuals with complex behavioral and physical health needs through assessment, care planning, crisis intervention, and care coordination.

What You'll Do

  • Conduct comprehensive assessments of members' behavioral health, physical health, and psychosocial needs
  • Develop individualized care plans that promote improved health outcomes and overall well-being
  • Collaborate with providers, care teams, and community resources to coordinate appropriate care
  • Apply clinical judgment to identify and address risk factors, barriers to care, and complex health needs
  • Utilize Motivational Interviewing techniques to engage members and encourage long-term behavior change
  • Provide crisis intervention and ensure timely access to appropriate treatment and support services
  • Coordinate behavioral health and substance use disorder services across the continuum of care
  • Leverage technology and clinical systems to document care and manage member interactions

WHAT YOU BRING - REQUIRED

  • Master's Degree in Social Work, Counseling, or a related Behavioral Health field
  • Active, unrestricted independent Behavioral Health license (LCSW, LISW, LMSW, LCPC, LPC, LMFT, LMHC, or equivalent)
  • 3+ years of post-master's direct clinical experience in hospital, ambulatory, outpatient, or similar behavioral health settings
  • 3+ years of experience treating mental health and substance use disorders
  • 3+ years of experience in crisis intervention and using Motivational Interviewing techniques
  • Strong technical proficiency and ability to document while engaging members telephonically

Preferred Qualifications

  • Managed Care or Utilization Review experience
  • Case Management experience
  • Discharge Planning experience
  • Experience working with complex populations and multidisciplinary teams

Pay Range

The typical pay range for this role is:

$60,522.00 - $129,615.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
