Our home health census is growing, our therapy team is expanding, and we're looking for talented Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) who know how to make the magic happen in the home.

Requirements

What You'll Do

As a member of our therapy team, you'll help patients improve independence with daily activities, increase safety, regain functional skills, and remain successfully at home.

OTs will:

  • Complete comprehensive evaluations and identify functional limitations.
  • Develop individualized plans of care and measurable treatment goals.
  • Assess ADLs, cognition, strength, coordination, safety, and home environment.
  • Reassess patients and modify treatment plans as clinically appropriate.
  • Recommend appropriate adaptive equipment and home modifications.

OTs & COTAs will:

  • Provide skilled occupational therapy in accordance with physician orders and CMS Conditions of Participation.
  • Provide ADL/IADL retraining, therapeutic exercise, functional mobility, balance, coordination, and upper-extremity interventions.
  • Educate patients and caregivers on fall prevention, energy conservation, adaptive techniques, and home safety.
  • Train patients in the safe use of adaptive equipment and durable medical equipment.
  • Maintain timely and accurate documentation.
  • Collaborate with physicians, caregivers, and the interdisciplinary team to support successful patient outcomes.

Our Ideal HHC Guru

You may be our next Home Health Magic Maker if you have:

  • Degree from an accredited OT or OTA program.
  • Current Michigan OT or OTA license in good standing.
  • COTA certification, as applicable.
  • Home health experience preferred.
  • Strong clinical judgment and problem-solving skills.
  • Excellent communication and patient education skills.
  • Ability to work independently while remaining connected to the care team.
  • Reliable transportation, valid driver's license, and current auto insurance.
  • Familiarity with Medicare/CMS documentation preferred.
