Job description

Job Title: Physical Therapist (PT) / Occupational Therapist (OT)
Location: Palm Beach County, FL
Position Type: Per Diem – Home Health

We are a rapidly growing homecare agency seeking licensed PTs and OTs to provide skilled therapy services to patients in their homes throughout Palm Beach County.

Responsibilities:

  • Perform initial evaluations and follow-up visits

  • Develop and implement individualized treatment plans

  • Provide skilled therapy to improve mobility, strength, and daily functioning

  • Educate patients and caregivers

  • Complete accurate and timely documentation

  • Communicate with physicians and interdisciplinary team

Requirements:

  • Active Florida PT or OT license

  • Home health experience preferred

  • Current CPR certification

  • Reliable transportation

  • Authorized to work in the U.S.

  • Updated physical and required in-services

Flexible per diem schedule. Pay is based on visit type and patient insurance.
