The Neighborhood Physician provides primary care for high-acuity Medicare Advantage patients within a dedicated panel, functioning as the primary care provider of record. This role partners with an interdisciplinary team to manage complex, high-utilization patients through a combination of clinic-based and home-based care. Care is delivered in a referral-based model, focused on reducing hospitalizations, improving outcomes, and supporting longitudinal care management.
The physician collaborates with advanced practice clinicians, case management, and social work to address medical and psychosocial needs, while supporting quality initiatives including HEDIS, RAF accuracy, and patient experience.
Primary Responsibilities:
• Manage a panel of complex Medicare patients as the PCP, overseeing all aspects of care.
• Provide care in clinic and home settings as appropriate, with APC support for field-based visits.
• Collaborate with interdisciplinary team (APCs, RN case managers, social workers) to optimize care plans and transitions.
• Review and coordinate care for high-risk patients, including post-discharge follow-up and chronic disease management.
• Partner with clinical quality teams to meet HEDIS, RAF, and documentation standards.
• Maintain accurate, timely documentation in the EMR.
• Participate in team meetings and contribute to care model development and workflow optimization.
• Provide clinical guidance and collaboration with APCs (independent practice model; not supervisory except where required).

• Flexible scheduling: 5x8 or 4x10 option; open to reduced FTE.
• Average volume: ~13-18 patients per day depending on schedule, with extended visit times for new patients.

Required Qualifications:
• MD or DO, Board Certified/Eligible in Internal Medicine or Family Medicine
• Active Nevada medical license and DEA (or ability to obtain)
Preferred Qualifications:
• Experience managing complex, high-acuity or medically frail populations preferred
• Strong clinical judgment with ability to manage complexity independently
• Team-based care collaboration and communication
• Comfort with value-based care, quality metrics, and documentation accuracy
• Interest in longitudinal care management and population health
Compensation for this specialty generally ranges from $237,500 - $384,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
