Department: Primary Care
Location: Muncie, Indiana
Reports To: Chief Clinical Officer/ VP of Operations
Sign-On Bonus:
  • $10,000 for Physicians (MD/DO)
  • Up to $5,000 for Nurse Practitioners or Physician Assistants with 3+ years of experience

Position Overview
Open Door Health Services is seeking a dedicated and compassionate Primary Care Provider (Physician, Nurse Practitioner or Physician Assistant) to deliver high-quality, patient-centered care. This role involves managing acute and chronic medical conditions while collaborating with an interdisciplinary healthcare team to support whole-person care.
As a Federally Qualified Health Center (FQHC), Open Door Health Services provides comprehensive primary, urgent, preventive, and select specialty services, along with social support programs for individuals and families—regardless of their ability to pay. Our patient population reflects diverse backgrounds, life experiences, and healthcare needs.

This position is located in Muncie, a mid-sized city in East Central Indiana. Recognized as one of the most affordable college towns in the United States (Realtor.com,) Muncie boasts top-tier arts and culture, entertainment, recreation, and education for all ages.

Roles and Responsibilities
Successful candidates will have:

  • Knowledge of clinical documentation and diagnostic standards (ICD)

  • Comfort using electronic health records (EHRs) and digital assessment tools
For Physicians (MD/DO):
  • Unrestricted license to practice medicine in the State of Indiana
  • Federal DEA certification and Indiana CSR
  • Board Certified or Board Eligible
  • Medical degree from an accredited medical school
  • Eligibility for unrestricted privileges at Ball Memorial Hospital
  • Strong community health/public health orientation
For Nurse Practitioners (NP):
  • Active Indiana Registered Nurse (RN) license
  • Indiana Nurse Practitioner authorization
  • Federal DEA certification and Indiana CSR
  • Graduate of an accredited school of nursing (BSN required)
  • Completion of an accredited Nurse Practitioner program with certification
  • Community health/public health experience preferred
For Physician Assistant (PA):
  • Possession of Indiana State Physician Assistant License
  • National Commission on Certification of Physician Assistant (NCCPA)
  • Federal DEA certification and Indiana CSR
  • Graduate of an accredited PA program required.
  • Successful completion of an approved Physician Assistant program, testing, and certification
  • Community health/public health experience preferred
