Healthcare United is seeking a Registered Nurse/Case Manager for a comprehensive home-based health care company committed to providing at-home services in the comfort of the client’s home. Locations in Stratford, Hamden, Norwalk, and Middletown.Full-Time/Part-Time - Flexible Hours - Choose 4/40 or 5 days! Per-Diem - Must Case Manage 10 patients and complete at least 5 assessments per month.Qualifications and Skills:Graduate of an accredited Registered Nurse program Current license as a Registered Nurse in Connecticut Ability to assess and respond to needs of patients and families in varied settings.
