Certified Nursing Assistant (CNA) No nights. No weekends. Tuition reimbursement for LPN/RN path.

The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team!! As a Hospice CNA, you'll work alongside an experienced interdisciplinary team to help patients maintain dignity, comfort, and quality of life in their homes and care facilities. Schedule, Hours & Territory

• Serving patients within approximately a one-hour of Branch

Qualifications Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.

• Observe, document, and promptly report changes in patient condition to the supervising nurse.

• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.

• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.

• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active certified nursing assistant (CNA) certification

• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.

• Ability to pass a criminal background check and motor vehicle record (MVR) screening.
