About ThedaCare!

Summary :The Trauma Social Worker provides as-needed crisis response and support to patients and families for trauma patients at ThedaCare Regional Medical Centers in Appleton and Neenah, as well as Children's Hospital of Wisconsin Fox Valley. Performs psycho-social assessment and develops a support plan that includes sensitivity to the cultural and special needs of each situation. Collaborates with interdisciplinary and community resources to meet the assessed needs.Job Description:

Schedule:

80-100 Hours per Month of On-Call

After Hours between 4:00pm - 7:30am On Call

24hr coverage Saturday, Sunday, and Holidays

KEY ACCOUNTABILITIES:

  • Provides on-site response within 60 minutes of being called for trauma/crisis support.

  • Serves as a liaison between hospital team members, patients, families, community health, and social and governmental agencies.

  • Provides and documents in the patient's electronic medical record a psycho-social assessment that utilizes critical thinking skills appropriate to multiple age groups, family dynamics, diverse populations, etc.

  • Develops and implements a plan for support based upon the needs of the psycho-social assessment.

  • Provides adaptability and continuing assessment concerning evolving crisis process and support needs.

  • Provides grief and bereavement support facilitation.

  • Provides collaborative support for team members concerning critical situations such as mandatory reporting situations.

  • Provides collaborative communication with hospital team members including successive handoffs of care after their shift.

QUALIFICATIONS:

  • Bachelor' degree in social work or counseling as required by the State of Wisconsin for certification

  • One year as a professional social worker or related field

  • Current licensure or certification in Wisconsin

PHYSICAL DEMANDS:

  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:

  • Frequent transition from standing, sitting with movement throughout the facility

  • Possible exposure to sharp objects and instruments

  • Possible exposure to fumes or airborne particles and toxic or caustic chemicals and radiation hazards, as well as blood and body fluids and pharmacological agents; therefore, personal protective equipment must be worn as necessary

  • Occasional high noise level in work environment

  • Possible contact with aggressive and or combative patients

  • Frequent use of keyboard with repetitive motion of hands, wrists, and fingers

Position requires compliance with department specific competencies.
