Develop, implement, review, and update nursing components of the individualized plan of care.

Communicate changes in patient condition promptly to the appropriate hospice team members and provider.

Participate in interdisciplinary group/team meetings and care-plan reviews.

Provide appropriate hospice aide supervision and ensure aide care is consistent with the patient's plan of care.

Complete nursing documentation accurately and within agency-required timeframes.

Assist with admissions, recertifications, transfers, discharges, deaths, and other hospice clinical processes as assigned.

Provide patient and family education regarding hospice services and end-of-life care.
