POSITION SUMMARY
The Registered Nurse (RN) - On-Call Nights & Weekends is responsible for delivering critical hospice nursing care during non-business hours. This role ensures timely clinical response to urgent patient needs overnight, on weekends, and on holidays, helping to maintain continuity and quality of care throughout the entire hospice experience.
This position is ideal for an experienced hospice nurse who is passionate about patient comfort and can confidently provide care independently during on-call hours.
PRIMARY RESPONSIBILITIES
  • Respond to patient calls, triage needs, and deliver compassionate, timely hospice care.
  • Assess and manage symptoms including pain, shortness of breath, agitation, and more.
  • Provide skilled nursing care in accordance with the plan of care and hospice protocols.
  • Administer medications and perform treatments as prescribed by the physician.
  • Communicate and collaborate with the interdisciplinary hospice team to ensure quality, coordinated care.
  • Support families and caregivers through education, emotional support, and guidance.
  • Accurately document all interactions, assessments, interventions, and changes in condition.
  • Travel to patient residences (e.g. Fresno Guest Homes or other in-home locations) to deliver in-person care when necessary.
  • Participate in hospice team meetings and ongoing staff development.

QUALIFICATIONS
  • Active Registered Nurse (RN) license in the State of California.
  • Minimum of one (1) year experience in hospice or palliative care preferred.
  • Certification in Hospice and Palliative Nursing (CHPN) is preferred, but not required.
  • Strong clinical assessment skills, including ability to make autonomous decisions in the field.
  • Demonstrated empathy and communication skills with patients and families in emotional or end-of-life situations.
  • Reliable transportation, valid driver's license, and proof of auto insurance (travel is required).
  • Comfortable with electronic documentation and use of EMR systems.
  • Bilingual in Spanish, Hmong, or other languages spoken by our patient population are a plus.

Work Schedule
  • Clocked In 5pm-9pm: In Person in facility visits
  • Unpaid 9pm-9:30pm Meal Period
  • Clocked In 9:30pm-1:30am: Office or home Paperwork / Documentation
  • (Clock In when called) 1:30am-6:00am: On Call Response as needed
  • Guaranteed 8hrs/day + any calls between 1:30am-6am
