Join a leading Home Health team providing compassionate, patient-centered care to adult and geriatric patients throughout the Central Metro E Chicago coverage area. This role offers flexibility with a primarily remote workflow while conducting in-home patient visits within the assigned territory.

Current Team Census: Approximately 200 patients
Traveler can expect approximately 2 days onsite in Oak Brook, IL for orientation and onboarding. After orientation, the role is fully remote/field-based within the coverage area.
Minimum 2 years of Home Health PT experience
Adult and geriatric patient experience required
First-time travelers considered for the right candidate
1-year experience candidates considered if recent Home Health experience
Active Illinois PT License
BLS (AHA)
Valid Driver's License
Active Auto Insurance
Reliable Transportation
Adult and geriatric general Med/Surg patients
Candidates must have experience with:
Assisting patients with ADLs (Activities of Daily Living)

Home Health patient assessments and interventions
