Job description

General: Full-time position (exempt) at forty (40) hours per week. Compensation is based upon 
qualifications and previous experience. Full benefits included.
Reports to: Nurse Practitioner (and working closely under the Programs Manager)
Defined:
The Registered Nurse (RN) at the Mosaic Care Center plays a vital role in the delivery of 
affirming, high-quality primary care for all patients, with a focus on LGBTQ+ individuals, people 
living with HIV, and other underserved communities. Under the supervision of the Nurse 
Practitioner, the RN provides direct patient care, care coordination, and clinical support across 
FAHASS’s integrated health and wellness services. The RN also supports programmatic goals 
under the direction of the Programs Manager, including Ryan White compliance and linkage to 
additional FAHASS services.
Employment Standards:
● Current, unencumbered Registered Nurse (RN) license in the Commonwealth of Virginia, 
or eligibility for licensure by endorsement.
● Associate’s Degree in Nursing (ADN) required; Bachelor’s Degree in Nursing (BSN) 
preferred.
● Current Basic Life Support (BLS/CPR) certification required.
● Minimum of one (1) year of clinical nursing experience, preferably in a primary care, 
federally qualified health center (FQHC), or community health setting.
● Experience working with people living with HIV/AIDS, LGBTQ+ individuals, or other 
medically underserved populations strongly preferred.
● Familiarity with Ryan White programs and eligibility requirements preferred.
● Bilingual (English/Spanish) a benefit.
Key Responsibilities Include the Following:
Patient Scheduling & Appointment Preparation
● Verify patient insurance coverage the day before each scheduled appointment, 
confirming active coverage, co-pays, and any prior authorization requirements.
● Conduct patient appointment reminder calls the day before each scheduled 
appointment, ensuring patients have transportation, understand preparation 
instructions, and are aware of any required documentation.
Care Coordination
● Provide comprehensive care coordination services for an assigned patient panel, 
including one or more of the following:
○ Telephone outreach to patients or caregivers to support health management and 
appointment adherence.
○ Medication reconciliation and adherence support, including patient education on 
prescribed regimens.
○ Coordination with outside providers, specialists, pharmacies, and laboratories to 
ensure continuity of care.
○ Review of external medical records and lab results, with appropriate 
documentation and communication to the clinical team.
○ Referral management and follow-up, ensuring timely completion and 
documentation of specialist referrals.
○ Care plan review and revision in collaboration with the Nurse Practitioner and 
multidisciplinary team.
○ Transitional care coordination for patients returning from hospitalizations, 
emergency department visits, or specialty care.
○ Communication with home health agencies, community services, and social 
services to address patients’ social determinants of health. 
● Assist with prior authorizations of medication and medical insurance communications.
Clinical Duties
● Perform venipuncture and specimen collection for laboratory testing; prepare and 
process specimens per clinic protocol.
● Assist with patient rooming, vital signs, medication administration, and clinical 
documentation in the electronic health record (EHR).
● Administer vaccinations and injections per provider orders and established clinical 
protocols.
● Perform point-of-care testing (e.g., rapid HIV, STI, glucose, pregnancy) per standing 
orders.
● Perform wound care, dressing changes, and other nursing procedures as ordered by the 
Nurse Practitioner.
● Conduct patient health education, including medication instructions, chronic disease selfmanagement, and preventive health guidance.
● Monitor patients for adverse reactions to medications or treatments and report concerns 
to the Nurse Practitioner promptly.
● Assist with in-office procedures as directed by the clinical team.
● Perform other clinical duties as assigned by the Nurse Practitioner.
Ryan White & Program Eligibility
● Confirm and document Ryan White eligibility for applicable patients, ensuring timely 
completion of required eligibility documentation and recertification.
● Screen patients for eligibility for additional FAHASS services, including pharmacy services, 
Status Neutral Service Navigation (SNSN), transportation, and other Ryan White-funded 
support services.
● Assist with data collection and documentation to support grant reporting and compliance 
requirements.
Quality, Safety & Compliance
● Maintain compliance with all HIPAA, OSHA, clinical, and organizational policies and 
procedures.
● Participate in quality improvement initiatives, including chart audits, patient satisfaction 
efforts, and performance measure tracking.
● Maintain a clean, organized, and stocked clinical environment; monitor and manage 
supply inventory.
● Follow all infection control and universal precautions protocols.
Team & Programmatic Responsibilities
● Collaborate with the multidisciplinary care team, including case managers, social 
workers, behavioral health staff, and community health workers, to address patient 
needs holistically.
● Attend and participate in team meetings, case conferences, and staff trainings.
● Perform other programmatic duties as assigned by the Programs Manager in support of 
FAHASS’s mission and grant requirements.
Additional Requirements:
● Demonstrated commitment to affirming, patient-centered, culturally responsive care for 
LGBTQ+ individuals, people living with HIV, and other underserved communities.
● Excellent verbal and written communication skills; ability to communicate effectively with 
diverse patient populations.
● Proficiency with electronic health record (EHR) systems; experience with AthenaHealth 
or similar system preferred.
● Ability to organize and prioritize multiple tasks while maintaining accuracy and attention 
to detail.
● Possession of a valid state driver’s license may be required for some activities.
● Occasional evening or weekend availability may be required for outreach events, 
trainings, or organizational needs.
● Ability to maintain confidentiality at all times and work effectively within a team 
framework.
● Perform other duties as requested by the Executive Director in support of FAHASS 
policies and mission.
