  • Certificate Copy (Fundamentals of Coaching Education, Concussion, Sudden Cardiac Arrest, Heat Illness Prevention, Bullying, Hazing and Inappropriate Behaviors)
  • CPR/First Aid Certification (Must have valid CPR & First Aid Certificate)
  • Letter of Introduction
  • Letter(s) of Recommendation (Must Submit Two (2) Letters Dated within the last Twelve (12) months.)
  • Proof of HS Graduation (or Equivalent)
  • Resume
