Job description

Schedule: Full-time| Monday - Friday | 8:00 a.m. to 4:30 p.m.
Pay Range: $104,000 to $120,000 (salaried exempt), based on experience, qualifications, and location.

As a Director of Home Health, you'll serve as both a strategic leader and clinical guide-driving the delivery of exceptional in-home care while advancing operational excellence across Vivie's footprint. In this Administrator-level role, you'll ensure compliance, foster team growth, and lead initiatives that expand access, improve outcomes, and elevate the patient and family experience. Your leadership will shape how we grow, serve, and innovate in home-based healthcare.
As a Director of Home Health, you will:
Oversee Home Health Operations
  • Provide day-to-day oversight of multiple sites while shaping long-term growth through strategic planning, budgeting, and care delivery alignment.

Ensure Regulatory & Clinical Excellence
  • Serve as the Administrator for home health agencies. Lead survey readiness efforts, manage MDH surveys, and drive consistent compliance with all federal, state, and internal standards.

Lead & Develop High-Performing Teams
  • Hire, coach, and support managers and care teams. Foster a strong, mission-driven culture with clear expectations, continuous education, and a commitment to quality.

Advance Growth & Community Partnerships
  • Collaborate with the sales team to drive inquiries, admissions, and community engagement. Ensure excellent communication with referral sources and successful service recovery.

Drive Quality Improvement Across Care Delivery
  • Partner with quality leaders to implement clinical benchmarks, explore care trends, and improve outcomes through sustainable, scalable processes.

Other Duties as Assigned
  • The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.

This job also requires:
  • Current licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist in Minnesota.
  • Minimum 1 year of supervisory or administrative experience in home health or related field.
  • Knowledge of home health regulations, standards of care, and survey processes.
  • CPR certification (or willingness to obtain within 6 months of hire).
  • Valid driver's license and insured personal vehicle.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with team members, visitors, and leaders.
