Work Environment

This position is onsite at our outpatient office, where you will work Monday to Friday, with potential weekend shifts as needed. You will work alongside other healthcare professionals, contributing to a dynamic and productive patient care environment, with a target patient care time of 75% or more.

Key Responsibilities
  • Assess and develop individualized treatment plans for patients struggling with various physical impairments, including injuries and conditions affecting muscles, nerves, joints, and bones.
  • Implement therapeutic interventions aimed at restoring function, alleviating discomfort, and preventing future debility.
  • Monitor patient progress and adjust treatment plans as needed to meet recovery goals, ensuring optimal patient outcomes.
  • Collaborate effectively with a team of therapists and healthcare professionals to facilitate patient care pathways and transitions, particularly for those preparing to move to a longer-term care environment.
  • Maintain comprehensive documentation of all patient interactions, treatment plans, and progress notes to ensure continuity of care and compliance with quality assurance protocols.
  • Participate in multidisciplinary meetings to share insights and contribute to patient-centered care discussions.
  • Continuously enhance your knowledge and skills by staying updated on the latest practices and treatment modalities in occupational therapy.
Patient Population / Client Focus

You will be working with a diverse patient population that includes individuals of all ages recovering from acute and chronic conditions. Our focus is on empowering patients through therapeutic exercises, functional activities, and education, all tailored to enhance their quality of life and independence. Your role will directly impact their recovery journey, emphasizing evidence-based practices and compassionate care.

Qualifications
  • A Bachelorโ€™s, Masterโ€™s, or Doctoral Degree in Occupational Therapy from an accredited program.
  • Current state licensure or eligibility to obtain licensure in Connecticut, along with certification in good standing.
  • Basic Life Support (BLS), CPR, or AED certification is preferred and should be maintained throughout employment.
  • Excellent communication skills, both verbal and written, are essential for interacting with patients and team members.
  • Proficiency in using technology, including electronic medical records (EMR) and Microsoft Office Suite, to document patient care and progress.
  • Ability to work independently, while also being a proactive team member dedicated to fostering collaborative relationships.
