Job description

In-patient Physical Therapy

In-patient physical therapy at San Gorgonio Memorial Hospital is focused on facilitating early mobility, with emphasis on safe patient handling and quality of care, reducing hospital-induced deconditioning and rate of re-admission through comprehensive care including patient education, family training, patient specific discharge planning, and establishing appropriate continuum of care. Acute care therapists serve all areas from Emergency Department through the Intensive Care Units, Med-Surg, and OB units. The physical therapist will be responsible for establishing and maintaining a comprehensive, individualized interdisciplinary plan of care under physician orders, in an acute care setting, which includes performing evaluations, integrative treatment planning, setting patient-specific functional goals, performing follow-up assessments, and ongoing patient/family education; coordinate patient’s Plan of Care with other medical and rehabilitation staff; initiate, manage and participate in case conferences, utilization review meetings, and discharge planning in collaboration with case managers, establishing continuum of care post-discharge; provide physical therapy practice act specific high quality of care and maintain EMR in accordance with Physical Therapy departmental policies and procedures, TJC/CHQ standards, federal and state regulations, Medicare, Medi-Cal, and primary insurance standards; foster a constructive and supportive approach to department operations, understanding economic constraints and practices designed to conserve resources, promote professional and collegial relationships; participate in process improvement and quality improvement measures; provide appropriate supervision and support to physical therapy assistants; act as a clinical educator for students and other clinical staff.

Qualifications: Required: graduation from an accredited program of physical therapy, or foreign trained physical therapy with state board licensure. Current California Board of Physical Therapy licensure. BLS certification. Pay/benefits: estimated pay is based on qualifications (including but not limited to experience, education, certifications, and licensure where applicable). Benefits may include medical, dental, vision, basic life and AD, optional supplemental life (employee, spouse, and/or child coverage), flexible spending accounts (medical and dependent care), 403b retirement with dollar for dollar match up to 5%, paid time off, and more available depending on employment status.
