Shift Detail: Mon thru Friday 8am -4:30pm Location is Watertown office. Intermittent need for weekend coverage for additional pay or comp time. Requires strong understand of Oasis.

The OASIS Workflow Coordinator reports to the Director of Clinical Services and plays a key role in ensuring the accuracy, compliance, and quality of OASIS assessments. This position reviews OASIS documentation for regulatory and policy adherence, provides guidance to clinical staff on documentation and utilization practices, and supports optimal patient care outcomes.

The OASIS Workflow Coordinator collaborates with clinicians to review visit utilization, recommend appropriate adjustments, and identify potential overutilization or underutilization concerns. This role is responsible for ensuring timely completion of workflows within Homecare Homebase (HCHB), supporting quality and performance improvement initiatives, and helping maintain compliance with regulatory and payor requirements.

Working closely with interdisciplinary teams, the OASIS Workflow Coordinator analyzes documentation trends, supports action planning, and promotes continuous improvement across home health operations to ensure the highest standards of clinical quality and operational excellence.


Qualifications
Required 
  • Degree from an educational program with licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP)
  • Current Connecticut licensure in one of the specialty fields (RN, PT, OT, or SLP) in good standing
  • Minimum of 2 years of certified home health experience
  • Strong understanding of OASIS regulations, documentation, and compliance standards
  • English verbal and written communication skills, including computer usage and teaching skills
Preferred 
  • Bachelor's degree in Nursing or Master's degree in PT, OT, or SLP from an accredited educational program
  • OASIS Certification and/or Quality/Compliance Certification
  • 3+ years of certified home health experience
  • Experience with Homecare Homebase (HCHB)
  • Experience in quality reporting, data analysis, and performance improvement initiatives
Knowledge, Skills & Abilities
  • Knowledge of federal and state regulatory requirements
  • Ability to provide education and clinical guidance to staff
  • Experience facilitating quality improvement and performance improvement initiatives
  • Ability to design and develop performance measures
  • Strong analytical, organizational, and problem-solving skills
  • Excellent written and verbal communication skills
  • Ability and willingness to travel to branches as needed
  • Ability to travel within the United States when necessary
