Nutrition Assistant (Irondequoit Campus) - Full/per diem

St. Ann's Community is currently seeking an evening full time and per diem Nutrition Assistant to aid in the maintaining resident nutritional information, process diet orders, selective and pre-selected menus and phone calls as well as ensure personalized resident meal selections are executed in accordance with prescribed plan of care.

Job Description:
  • Manages dietary management software including but not limited to: admissions and discharges, updating resident preferences and profiles
  • Inputs resident/participant demographics, diet orders, food tolerances and preferences. Develops 21 permanent menu cycles for designated residents/participants. Updates all information as needed.
  • Distributes selective menus; assist residents in menu completion.
  • Collects, processes, and checks resident menus for compliance and adequacy within dietary restrictions.
  • Execute and audit quality assurance reports as requested by Nutrition Services Manager
  • Prints computerized nourishment tallies, nourishment labels, tray tickets and needed reports.
  • Separates tray tickets; combines tickets in order of cart assembly.
  • Implements food and fluid monitoring on designated residents.
  • Processes incoming phone calls, communicating messages and resident participant concerns to appropriate staff/work area.
  • Act as liason between residents, family members, interdisciplinary staff members, clinical nutrition, and dining services leadership staff to communicate resident diet needs
  • Maintains a high level of professionalism, uses critical thinking, and independent latitude for judgement when interfacing with residents, family members, and staff regarding clinical nutrition issues.
  • Performs other duties as assigned by Nutrition Services Manager
  • Perform other duties as required.
  • Reliable, dependable and punctual in use of work time.
  • Demonstrates initiative and cooperation in professional working relationships.
  • Complies with applicable organizational policies and procedures ie: mandatory education, health requirement, HR requirements.
  • Demonstrates a commitment of quality through participation on improvement teams and identifying opportunities for improvement.
  • Works to improve performance relative to the system's organizational goals.
  • Consistently delivers high quality service being responsive to customer needs.
Candidate Requirements:
  • High School Diploma or equivalency with 1 year experience in customer service preferred.
  • Requires computer proficiency, including proficiency in Microsoft Office.
  • Experience with CBORD or other nutrition management system preferred.
