Job Description:

1.Provides direct patient care in accordance with state regulated competencies depending on level of certification (I or II)
2.Provides safe, efficient and high quality personal care services to assigned residents under the supervision and guidance of a registered nurse, and in accordance with Hospice policies.
3.Adheres to written and oral instructions for patient care as prepared by the Nurse.
4.Assists the patient with all aspects of personal care, ambulation and positioning as required.
5.Prepares meals and assists with feeding as requested.
6.Performs housekeeping services that are necessary for the health and safety of the patient/family/caregiver.
7.Provides guidance to the family/caregiver regarding personal care of the patient.
8.Attends and participates in weekly team meetings, facility morning report, inservices, department, and agency meetings and other related activities.
9.Understands and adheres to the Facility's Mission and policies and procedures, and advocates its philosophy.
10.Follows the guidelines for universal precautions when providing patient care.
11.Demonstrates a positive attitude and working relationship with staff members and others.
12.Maintains records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by the Facility.
13.Answers telephone within three rings, identifies department and self, directs call to appropriate person or assists caller.
14.Answers patient's call bell within three rings.
15.Maintains strict confidentiality of patient/family/caregiver information.
16.Provides for a safe environment for patients, guests, visitors and employees.
17.Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management.
