Job description

Licensed Nurse Practitioner

We are looking to add a licensed Nurse Practitioner to our growing team of medical professionals. Your primary responsibility will be to provide advanced primary and/or specialty nursing care to patients in our facility. Duties will include but are not limited to, performing physical exams, recording patient medical histories, creating patient care plans, and administering diagnostic tests. You will also be administering medicines and treatments, as well as observing the patient response to such treatments.

A Master's Degree in Nursing and the required state license are necessary for you to be considered as a candidate. Previous experience in providing primary care and proficiency with the medical instruments and equipment required by the work are important keys to becoming successful in this role.

Responsibilities
  1. Demonstrate knowledge of the principles of growth and development of pediatric, adolescent, adult and geriatric clients served by BRPCC. Demonstrate the ability to apply these principles in assessing and interpreting client information. Demonstrate competency in planning and implementing individualized care plans based on the age-related needs of adult and geriatric clients.
  2. Effectively conduct comprehensive or episodic health assessments of adult and geriatric clients. Assessments may include the psychosocial, cognitive, and personal assets and limitations of the client. This function includes use of judgment and independent acts not based on protocol. Emphasis is on wellness and harm reduction.
  3. Accurately establish medical diagnoses; order and evaluate diagnostic studies; perform and monitor therapeutic procedures; assess and/or manage follow-up plans.
  4. Effectively prescribe and regulate medications per written Physician Agreement.
  5. Accurately and legibly document health appraisal data necessary to maintain, coordinate and/or expedite required services.
  6. Appropriately adhere to BRPCC project, grantor, and governmental guidelines.
  7. Regularly act as a resource to health care staff and implement educational programs to improve care management of clients.
  8. Effectively develop, conduct a/or evaluate formalized in-service and continuing education programs in area(s) of expertise.
  9. Regularly seek to update and maintain a current level of knowledge relative to professional practice, and maintain continuing education requirements necessary for licensure.
  10. Acknowledge the importance of the research process by contributing ideas, collecting data, sharing knowledge and/or applying new findings to practice.
  11. Identify and secure resources to meet service needs of BPRCC clients and in the absence of community resources advocate for their development.
  12. Actively participate in performance improvement activities within the agency and Medical team.
  13. Actively participate in advocacy activities that support the mission of BRPCC.
  14. Appropriately perform other activities not described above that are deemed appropriate by the agency.
Education/Experience Requirements
  1. A Master of Science in Nursing with a specialty in family health.
  2. Professional knowledge of nursing theory and practice. Extensive knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology and management of acute and chronic client problems for adult and geriatric clients.
  3. Current licensure and certification required by the State of Louisiana to practice as a Nurse Practitioner including prescriptive authority.
  4. Substantial interpersonal skills necessary to instruct patients and any available family members and to collaborate with health team members.
  5. Comfortable working with homeless and/or low-income clients.
