Job Summary
We are a family-owned and operated Home Health and Hospice agency seeking a compassionate and dedicated Registered Nurse (RN) to join our growing team. We are looking for full-time, part-time, and PRN positions. In this role, you will provide skilled nursing care to patients in their homes while developing, implementing, and managing individualized plans of care that promote comfort, healing, and quality of life.
As a Home Health RN, you will serve as a patient advocate and trusted partner to families during some of life's most important moments. You operate with strong clinical judgment, exceptional attention to detail, and a genuine heart for service. You take pride in delivering personalized care, collaborating with interdisciplinary teams, and ensuring each patient receives care aligned with their goals and needs.
Responsibilities
  • Perform comprehensive patient assessments and develop individualized plans of care
  • Provide skilled nursing services in accordance with physician orders and agency policies
  • Manage and coordinate care for home health and hospice patients
  • Monitor patient progress and adjust care plans as appropriate
  • Educate patients and families on disease management, medications, and end-of-life care
  • Accurately document visits and maintain compliance with Medicare regulations
  • Collaborate with physicians and interdisciplinary team members
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employment
  • Previous experience as a Registered Nurse is preferred
  • Excellent interpersonal and client care skills
  • Valid driver's license
  • First aid/CPR certification required
