About the Position

Plans and provides health services, within the accepted scope of practice for nurse practitioners, for Goodwin House residents to promote a maximum level of health and independent functioning. Services include providing primary and episodic health care including services necessary for the promotion of health, the prevention of disease, and the early detection and treatment of illness.

Job Duties

  • Provides primary and episodic health care which includes services necessary for the promotion of health, the prevention of disease, the early detection and treatment of acute and chronic illness, and emergency care in this setting to residents at a Life Plan Community. This service may require collaboration among and referral to other health professionals, including but not limited to specialty physicians, physical, occupational, or speech therapists, social services, home health and hospice. This service will take place in the clinic, the resident apartment, in Assisted Living, or in the Health Care Center. This may include taking comprehensive health histories, performing physical examinations, performing or ordering appropriate lab tests, x-ray, and other tests, prescribing, and implementing therapy according to standards of practice.
  • Participates in and facilitates admission, transfer, and discharge of residents both within Goodwin House communities and between Goodwin House and community facilities by assessing the resident’s medical needs to ensure both the appropriate level of care and continuity of care.
  • Independently performs complete and episodic physical assessments on adult and geriatric populations. Independently orders, performs, and interprets laboratory data, including diagnostic procedures. Independently orders radiological studies. Independently orders physical, occupational, and speech therapy.
  • Prescribes drugs according to an approved formulary established by the Boards of Nursing and Medicare in the Commonwealth of Virginia.
  • Independently assesses the needs for and provides counseling to residents on issues such as health promotion, disease prevention strategies, medicine administration, hormonal replacement therapy, continence management.
  • Provides initial emergency assessment and care when necessary.
  • Completes accurate medical records in a timely manner on a daily basis, including completing accurate and timely chart entries on resident transactions, orders, test results, and progress notes summarizing resident care for continuity of care between visits.
  • Collaborates and validates with clinic team members on policies and procedures to facilitate the work of the resident clinic and to ensure consistency in practice.
  • Develops and refines clinic operations to include appointment scheduling, workflow, medical record composition, billing protocols, and all other areas pertinent to daily operation of the Clinic.
  • Participates in the admissions process, interviewing prospective residents, making health and medical recommendations to the admissions committee, and ensuring the admission health requirements are met.
  • Serves in a case manager role coordinating and communicating between resident, other departments and services within Goodwin Living, the physician(s), and community resources to facilitate comprehensive and continuous health care for the resident.
  • Serves as preceptor to nurse practitioner students.
  • Attends educational training to maintain a current knowledge and competency base and to maintain required Nurse Practitioner certification.
  • Establishes and follows a practice protocol with the Medical Director to meet the Virginia Board of Nursing requirements for Nurse Practitioners.
  • Participates in research activities for projects which are appropriate for the Goodwin House setting and which have been approved by an authentic Institutional Review Board for research. Is an active member of the Goodwin House Institutional Review Board.
  • Plans and facilitates health education programs for residents. Provides education to residents on health related matters via a variety of medium including Gazette articles, clinic wellness education activities, individual counseling.
  • Maintains membership in local and national professional organizations to maintain current knowledge base.
  • Participates in intra and inter department teams as assigned.
  • The nurse practitioner’s Medicare provider number will be the number used for reimbursement for clinic services provided by the nurse practitioner and all clinic team members. The nurse practitioner is responsible for the following:

a. Must meet and maintain professional requirements to qualify as Medicare and other insurance provider. This includes maintaining appropriate licensing, certification, collaborative and supervisory agreements with the Medical Director.

b. Provides nurse practitioner services and prepares appropriate level of documentation to meet requirements for correct insurance reimbursement.

c. Provides professional oversight of clinic team members, ensuring an acceptable standard of care is maintained and requirements are met for insurance billing and reimbursement.

Job Requirements:

  • Master of Science Degree in Nursing as Geriatric, Adult, or Family Nurse Practitioner.
  • Professional Certification consistent with the specialty area (Geriatric, Adult, Family) of NPs educational program and maintains certification.
  • Current license as an RN and NP in the state of Virginia.
  • Maintains current federal DEA number along with nurse practitioner prescriptive privileges and attends continuing education offerings to meet the requirements for these licenses.
  • Current CPR and First Aid certification.
