Job Overview: As a Nurse Practitioner in the Home Health unit, you will play a vital role in providing comprehensive healthcare services to patients in their homes. This fee-for-service position involves serving as a NY Medicaid Provider and requires ANCC certification. Join a dynamic team dedicated to delivering high-quality home health care in the comfort of patients' homes.

Key Responsibilities:

  • Conduct patient assessments, diagnose medical conditions, and develop appropriate care plans.
