The LCSW provides advanced clinical assessment and leadership in managing complex psychosocial cases. You would ensure effective care planning, crisis intervention, and seamless coordination across the continuum of care.
This role reports to the Director.

Essential Job Duties:

Conducts home and facility visits to evaluate patients and families in non-hospital settings
Collaborates with Clinical Care Coordinators to identify and manage complex cases requiring psychosocial intervention
Performs and documents depression screenings and ensures appropriate follow-up care
Completes follow-up calls or visits within one business day of care encounters
Works with Medical Management to interpret and apply beneficiary benefits and services as a liaison
Communicates and collaborates with hospital social workers to determine appropriate transition of care needs
Performs comprehensive psychosocial assessments with early identification of complex issues or changes in condition
Incorporates age-specific needs into care planning
Effectively manages workload to ensure timely and smooth care transitions
Assumes a leadership role in managing catastrophic cases and executing care plans
Facilitates and coordinates transitions for complex patients, including arranging transportation as needed
Maintains knowledge of facilities and appropriate community resources
Applies federal, state, and organizational regulations related to care planning and continuum of care
Communicates care plans with Clinical Care Coordinators, physicians, families, and care facilities
Maintains accurate, concise, and timely documentation in medical and departmental systems
Participates in departmental, ACO, and community education and in-service programs
 

Qualifications:

Master’s degree in Social Work required
Minimum of five years of clinical experience within the last seven years
Strong knowledge of clinical social work practices and Indiana state laws
Excellent verbal and written communication skills
Experience in case management preferred
Ability to manage complex cases and collaborate across interdisciplinary teams
Strong organizational, documentation, and time management skills
 

Licenses / Certifications:

Current Social Work licensure through the State of Indiana required
Basic Life Support (BLS) certification required
