Position Summary
The Registered Nurse (RN) provides skilled nursing care to patients under the direction of a physician. Responsibilities include implementing and evaluating individualized patient care plans, coordinating with care teams, and ensuring quality care delivery. The RN may supervise LPNs, CNAs, and HHAs, and retains responsibility for the overall nursing care of assigned patients.
PRN available to provide Home Health in Duval, St. John's, Clay and Nassau counties in FL.
Competitive visit rates and mileage offered.
Essential Functions
  • Perform start-of-care visits, evaluations, routine visits, and discharge visits.
  • Develop and implement a nursing plan of care based on physician orders.
  • Monitor, observe, and report symptoms, reactions, and condition changes.
  • Provide preventative and rehabilitative nursing procedures.
  • Document all care services at the time of visit and maintain compliance with all clinical documentation requirements
  • Counsel patients and caregivers on nursing needs.
  • Assign and supervise care tasks to LPNs, CNAs, or HHAs, retaining responsibility for care.
  • Participate in interdisciplinary case conferences and communicate relevant patient updates to care teams.
  • Participate in on-call rotations and in-service training events.
  • Represent the agency professionally in all patient and community interactions.
  • Uphold confidentiality and adhere to HIPAA and infection control protocols.
  • Support continuous quality improvement and care delivery standards.
  • Perform additional duties as required.

Qualifications
Knowledge and Critical Skills
  • Strong communication and listening abilities.
  • Ability to work independently while contributing to team goals.
  • Proficient in Microsoft Office (Word, Excel, Outlook, PowerPoint).
  • Ability to adapt to diverse care settings and patient needs.

Experience
  • Clinical experience appropriate to licensure; home health experience preferred.

License / Certification
  • Current Florida Registered Nurse (RN) license.
  • Valid Florida driver's license with acceptable driving record, reliable transportation and current auto insurance.
  • Current CPR certification.

Education
  • Bachelor's degree in Nursing preferred; Associate degree acceptable with appropriate experience.

Other Requirements
  • Bilingual Spanish skills are a plus.
  • Ability to work in various settings, including patient homes, facilities, and mobile environments.
  • Capable of working under pressure and in variable environmental conditions.
  • Current TB testing and documentation of immunizations or declinations (Hepatitis B and Flu).
  • Level II AHCA background screening clearance
