Registered Nurse or Licensed Practical Nurse Home Health Position Details
The RN or LPN Case Manager provides skilled nursing care, patient education, and care coordination in collaboration with the healthcare team to support optimal patient outcomes.
  • Assess patient and family needs and develop individualized care plans
  • Deliver skilled nursing care and health teaching
  • Monitor patient condition and report significant changes
  • Maintain accurate documentation of visits and care provided
  • Coordinate care with physicians and interdisciplinary team
  • Supervise Home Health Aides and support team collaboration
  • Participate in case coordination, quality improvement, and staff support
  • Assist with staff training, onboarding, and performance evaluation
  • Perform additional duties and leadership responsibilities as assigned
