Play a key role in delivering exceptional care by conducting client assessments, shaping personalized care plans, and ensuring caregivers consistently meet the highest standards. In this PRN role, you’ll lead caregiver onboarding and orientation, guiding new hires through hands-on competency and skills evaluations that set them up for success.

PRIMARY RESPONSIBILITIES

 

  • Travel to clients’ homes to conduct nursing assessments and document observations
  • Evaluate and update care plans for clients and caregivers
  • Perform regular and as-needed client assessments to ensure care aligns with evolving needs
  • Use clinical judgment to identify physical, mental, and psychosocial needs and develop comprehensive, multidisciplinary care plans
  • Complete follow-up assessments in accordance with state and company policies
  • Collaborate with management to implement and refine procedures that support high-quality client care
  • Serve as a resource and mentor to staff, upholding professional nursing standards
  • Advocate for client rights and ensure they are respected at all times
  • Maintain awareness of home safety conditions and promote a strong culture of safety
  • Lead and support caregiver onboarding, training, and orientation through competency-based skills evaluations
  • Coach caregivers on proper procedures and reinforce safety and care standards consistently

Qualifications

  • Work effectively as a collaborative team member
  • Maintain strict confidentiality at all times
  • Communicate clearly and professionally with staff, clients, families, referral sources, and community partners
  • Demonstrate strong problem-solving and critical thinking skills
  • Provide direction, instruction, and support to caregivers while on duty

  • Manage multiple priorities while working both independently and efficiently

Required Education and Experience

 

  • Associate Degree in Nursing or Bachelor of Science in Nursing
  • Current, unencumbered Maryland Registered Nurse license
  • CPR Certification
  • Current TB Test
  • Must maintain CEU requirements as required by the Board of Nursing
  • Reliable transportation
