FirstHealth Hospice & Palliative Care is a 5 STAR rated non-profit hospice offering compassionate end-of-life care support.

As a Hospice RN Case Manager you will have flexibility in providing skilled complex care to the hospice patient and family in the community. The RN case manager will work collectively with members of the interdisciplinary team to facilitate and oversee a care plan and symptom management needs in the home environment. The complexity of managing and educating the hospice patient & family on advanced disease processes is a rewarding experience and opportunity to practice in a specialty area.
