Job description

Role Summary

Our Patient Navigators partner with our Care Team in helping our patients grow their families! Our Patient Navigators work hand-in-hand with our nursing team to provide our patients with the information they need to approach their treatment cycle with confidence. They simplify an often difficult and confusing process to ensure a seamless transition to the nursing team for cycle management.

Some day-to-day responsibilities include:

  • Generate journey maps and calendars based on treatments plans
  • Facilitate, track and document completion of tests and diagnostic procedures
  • Develop trust and positive rapport with all new patients
  • Explain complex medical process and terms in a digestible manger
  • Monitor patient compliance and readiness for treatment
  • Scheduling appointments

This is a full-time non-exempt onsite position that requires early hours, rotational weekends and holidays. 

Compensation

The starting salary range for this role is $23-27/hr. Salary is based on several factors including job related knowledge and skill set, depth of experience, certifications and/or degrees.
