Job description

Job Title: Social Worker - MSW / LSW
Location: Methodist Hospital
1633 N Capitol Ave. Suite 680, Indianapolis, IN 46202
Shift: Full-Time Days, 8:00 am - 4:30 pm Mon-Fri

 
Position Overview: Provides face-to-face psychosocial assessments, resource coordination for patients and families. Collaborates with the healthcare team to ensure safe, timely care and effective resource utilization. This position does psychosocial evaluations, assists in advanced care planning, and connects patients to resources.

 
Key Responsibilities:  
Meet face-to-face with patients and families to assess psychosocial needs and risks.  
Coordinate resources for safe patient transitions.  
Collaborate with all healthcare team members to facilitate effective care and resource delivery.  
Interpret and communicate complex patient and family needs, role modeling caring practices.  
Counsel patients and families on emotional, social, and financial impacts of illness or disability.  
Access and mobilize community and family resources to meet identified needs.  
Support de-escalation, and safety planning in high-risk situations.  
Conduct interviews with victims, perpetrators, and significant others, ensuring safety and legal compliance.  
Document all clinical activities accurately in the electronic medical record system.  
Develop and maintain strong working relationships with hospital staff, law enforcement, community agencies, and protective services.  
Respond to or support responses with the Department of Public Safety to de-escalate violence.

 
Qualifications & Requirements:  
Master's Degree in Social Work (MSW) required.  
Permanent licensure as an LSW in Indiana required.  
Certification in Crisis Prevention Intervention or an alternative de-escalation training within 6 months of hire.  
Knowledge of evidence-based psychosocial assessment for mental health and substance use disorders.  
Understanding of disease processes and human behavior related to health and social needs.  
Complex discharge planning skills for safe patient transitions.  
Knowledge of state and county legal mandates related to protective services.  
Understanding of growth and development principles across the lifespan, including end-of-life care.  
Ability to identify decision-makers and coordinate goals of care conversations.  
Experience adapting services for geographically, culturally, and financially diverse populations.  
Strong ability to develop and maintain collaborative relationships with hospital staff, law enforcement, community agencies, and external partners.  
Computer literacy and proficiency in electronic medical record documentation.  
Ability to apply principles of crisis intervention and de-escalation techniques.  
May be scheduled for on-call and holiday rotations.
